Immunization Requirements
Washington state immunization law, RCW28A.210.901 requires every child in public or private schools or in licensed day care facilities to be fully immunized unless exempt for medical, personal or religious reasons.  Medical exemptions require a physician’s signature.  Personal or religious exemptions require the signature of a parent or guardian.  A completed CIS form (Certificate of Immunization) must be on file before or on each child’s first day of school.

Administration of Medications at School
If a student requires medication at school, the following requirements must be met in accordance with WA State law RCW 28A. 31. 150 and 155. 

-         A doctor physician order form must be completed and signed by the child’s doctor, DO, ARNP, PA or dentist.  The bottom portion of the form must be completed and signed by the parent or guardian.

-         Prescribed medication must be in the original container labeled with the student’s name, name of medication, dosage and instructions.

-         Over the counter (OTC) medication must be in the original, unopened container.

Important notes for medications at school:

-         OTC (over the counter) medications such as Tylenol, Ibuprofen, Motrin, cough syrup, Benadryl, etc. require a completed physician order form.  NO medication will be given without a doctor’s order.  NO EXCEPTIONS!

-         All medications must be kept in the nurse’s office.  Students are not to carry medication on them.

-         Parents are welcome to come to school to give medications to their children when necessary.

-         Doctor’s orders expire at the end of each school year.  If a student is to receive the same medication the following school year, a new form must be completed.

Health Screening
1. Basic vision and hearing screening is done in the fall for grades K-6.  Lice screening is also done at this time.
2.  Scoliosis screening for curvature of the spine is done in the spring for grades 6 and 8.
3.  Blood pressure screening is done for high school students depending upon the availability of trained volunteers.

Head Lice
LCSA has a nit-free policy concerning lice.  Any student found to have live lice or viable eggs (nits) will be sent home.  After appropriate treatment at home the student must be cleared by the school nurse before returning to class.  All lice and all nits must be removed.  Information about products and lice removal from hair and homes is available through the nurse.

General Information
    
Sick/Injured Students

-         Students should be picked up within one hour of being called by the school nurse or office.  The health room is not equipped to take care of sick or injured students for extended periods of time.  Alternative arrangements should be made ahead of time if a parent/guardian is unable to pick up the student.

-         Students should be fever free without medication (below 100 degrees F) for 24 hours before returning to school.  Also they should not have had any vomiting for at least 24 hours before returning to school.

     Allergies

-         It is very important to let the nurse know if a child has a serious allergy which may require the administration of epinephrine.  A doctor’s order must be completed and the epi pen provided by the parent.  If a student is to carry an epi pen or asthma inhaler with him/her the doctor’s order must state so.  It is recommended that a spare inhaler or epi pen be provided and kept in the nurse’s office.

     Medical Conditions

           -    The school nurse is available to help with special health concerns or medical conditions.
